2005 Registration Information

Team Reps please check your forms to see that the common errors listed below are caught before submitting to the Office. Also, there is no Team Form this season, so please be sure all forms are legible! Individuals can download a member form in .pdf format and type in their information. See the Forms section of web site to download. New for 2004/2005 - we will have an online registration option. The Carolina Region is one of four regions beta-testing USA Volleyball's new online database and event management program. With this system, individuals will be able to register (or renew for previous members) online, keep the Carolina Region and USAV up-to-date on any changes in personal information (addresses, phone, email), view their club/team information, print membership verification, and much more. Club/Team Reps will eventually be able to view all the members in their club, create emails to go to their club/team members, enter events, and more. As this is a brand new system just created for USAV in the last several months and we are just beta-testing it this season, there are sure to be some bugs (we will keep our current offline database this year as backup). We appreciate everyone's help (and patience) in notifying us of anything that seems wrong on the system and helping us improve it for all our benefit. A lot of the capabilities will not be available from the start, but the member registration component is supposed to go live November 1st. Common errors with Registration:

• Member Forms not signed on Back in appropriate places. In addition, Juniors must sign the form and their parent/guardian must sign the form (on back)
• Missing birthdate or birthdate incorrect. Year of birth is often listed as current year (04 or 05).
• Team Rep not indicated or Team Division not checked on the Team Rep form.
• Juniors - Coaches' Code of Ethics not submitted for all coaches.
• Juniors - Background Screen forms not submitted for all adults (team rep, club rep, coaches, chaperones, trainers, etc) wanting to affiliate with the junior team.
• Copies or Faxed copies of member form is submitted. Our insurance company requires original signatures (in pen) on file in the office.
• Membership forms completed in Pencil. Please use an ink pen when completing any forms to be submitted.
• Juniors - reps send in Medical Release forms to Office. These forms are to be kept by the club and carried by the coaches to all competitions and practices. The Region does not need a copy of them.
